Sec. 3. Not later than the tenth day of each calendar month, the clerk of the court shall forward to the Indiana department of health records of decrees of:
(2) annulment, revocation, or amendment of adoption;
(1) adoption; or

- Amendment: A proposal to alter the text of a pending bill or other measure by striking out some of it, by inserting new language, or both. Before an amendment becomes part of the measure, thelegislature must agree to it.
- Clerk: means the clerk of the court or a person authorized to perform the clerk's duties. See Indiana Code 1-1-4-5
- Month: means a calendar month, unless otherwise expressed. See Indiana Code 1-1-4-5
entered in the preceding month, together with related reports required by the Indiana department of health.
