a health care facility as defined in Section 26B-2-201; or
(b)
the offices of one or more private prescribers, whether for individual or group practice. See Utah Code 58-88-201
Licensed dispensing practice: means a health care practice that is licensed as a dispensing practice under Section 58-88-202. See Utah Code 58-88-201
State: when applied to the different parts of the United States, includes a state, district, or territory of the United States. See Utah Code 68-3-12.5
(a)
“Dispense” means the delivery by a prescriber of a prescription drug or device to a patient, including the packaging, labeling, and security necessary to prepare and safeguard the drug or device for supplying to a patient.
(b)
“Dispense” does not include:
(i)
prescribing or administering a drug or device; or
(ii)
delivering to a patient a sample packaged for individual use by a licensed manufacturer or re-packager of a drug or device.
